Tailored to the stroke survivor 18
years and older
- Comprehensive rehabilitation for individuals who have suffered a stroke
- Intensive program and team of professionals
- Patients and families are integral members of the team
- Goal is to maximize functional independence and facilitate a return
to
the community
Scope of Services
The program provides specialized and
individualized treatment in all areas
of stroke rehabilitation. In
addition to the expert rehabilitative services
provided by the stroke
rehabilitation team, specialized medical consultations
are available.
Consistent communication is maintained throughout the continuum
of care,
including communication with the referring physician.
