What information does my physician need to provide to complete the referral?

Your doctor will need to provide:

  • Your demographics
  • Office notes with a history of your pain and reason for referral
  • List of current medications
  • Outline of previous medications, tests, X-rays, and treatments

Should I bring anything with me to my first pain management clinic appointment / consultation?

Please bring the following to your first appointment at our Pain Clinic:

  • Any medical records or X-ray reports you have related to your pain
  • Medications you are currently taking
  • Information you have relating to health insurance coverage

What happens on the day of my pain management procedure?

If you are scheduled for a procedure after discussion with the physician, you will receive specific education and instructions. Generally, most injections require you to have a responsible adult with you to drive you home after the procedure. If you do not have someone to drive you home, your procedure will need to be rescheduled. You can take medications and eat a light meal prior to your procedure unless otherwise instructed. We advise all patients to go home and rest for the remainder of the day after their procedure, do not return to work.

Please arrive at the time your procedure is scheduled. You do not need to arrive any sooner. After registration, a nurse will review any changes in your health, review your medications, obtain your vital signs, and answer any questions you may have prior to your injection. You will have a short recovery after the injection. Then a nurse will review your discharge instructions and provide details for any follow-up visits.
