What Sets Us Apart?

Pre-hab Visit

Our homecare rehab team will visit the patient’s home prior to surgery. This Pre-hab Visit ensures that the patient/family is well-prepared for return home post-operatively. The therapists assess and address safety issues throughout the home, make recommendations for modifications to make post-surgical life easier, recommend equipment that may be helpful after surgery and offer valuable education to ensure a safe and successful recovery. The outcomes of the Pre-hab Visit are reported to the hospital team to ensure optimal care coordination.

Multidisciplinary Team

Our team of specially trained nurses, physical therapists and occupational therapists all work together to create an individual care plan that is just right for you.

Our Goal.

Our goal is to get you back to your regular activities as soon as possible. We provide the best possible therapy options to do just that.

Once at home, your therapy will focus on mobility, safety, and pain management. You will be expected to participate in the prescribed exercise program on your own to speed healing. Once you are able to safely move about on your own (after approximately 2-14 days), you will transition to outpatient therapy.
